Outpatient Medical Care
Outpatient medical care focuses on delivering comprehensive HIV-specific health care and treatment to uninsured and medically under-served HIV+ persons, including Highly Active Anti-Retroviral Therapy. The ultimate goal of treatment is to reduce, manage and / or prevent physical complications associated with HIV disease. Professional, diagnostic and therapeutic services are provided by a nurse practitioner in an outpatient, community based, and / or office-based setting.
Case Management
The purpose of case management is to coordinate client access to services within the agency and in the community. Our team of Wellness Coordinators serve as the initial contact for clients requesting services, assist clients in connecting with primary health services and educate them about health issues and living with HIV.
Social Case Managers work with clients on a long-term basis to address dual diagnoses, mental health needs, and administer financial assistance programs for housing and health insurance.
Medical case managers ensure that clients have access to needed medical services and encourage adherence to prescribed treatment regiments. They coordinate with medical providers, provide the information, referrals and linkages needed for medical services, provide education about medical therapies and monitoring and following up on the goals of the care plan.
Whatever the need, case management strives to link clients with a solution.
Housing Assistance
Housing Opportunities for People with AIDS (HOWPA) provides for long term rental assistance and short term rental assistance includes the provision of rental, mortgage, and utility payments to a mortgagor. The purpose of this program is to prevent eviction or homelessness of an eligible client. This may include payment of rent, mortgage, and / or utility payments to a landlord, mortgage holder or utility provider.
Housing and Urban Development Program (HUD) programs include the provision of rental, mortgage, and utility payments to a mortgagor. The purpose of this program is to provide ‘permanent’ housing for people who are homeless and living with HIV. This program is designed to help eligible clients maintain permanent housing, achieve greater self-determination and increase their income. Comprehensive case management is available to eligible clients on the program.
Nutrition Center
The nutrition center provides food, personal hygiene products, paper products, and cleaning supplies on a bi-weekly or monthly basis. The aim of Nutrition Center services is to offset the high cost of living with HIV/AIDS and provide a well-rounded supplemental supply of groceries and personal hygiene products to assist in maintaining sound physical/nutritional health. In addition to supplies, we offer education and referrals for nutritional counseling to help clients develop healthy diet plans.
Volunteer Support
Clients and staff are assisted by volunteers in the center at the reception area, in the nutrition center, and with various clerical tasks.

HIV / STD Education
Whether in the classroom, the workplace, or other institution, ASNT has over a decade of experience in tailoring educational presentation to audiences for the purposes of increasing awareness of HIV and other sexually-transmitted diseases.
Our message is also geared toward helping people understanding the disease and alleviating fears and phobias about its transmission so as to create a more positive and accepting and compassionate environment for those who are living with the virus.

Counseling
ASNT's licensed professional counseling team provides individual and family therapy to clients of ASNT who are struggling with the emotional impact of HIV/AIDS. Counseling also offers opportunities to address issues that may be hindering clients from optimal disease management.
Receiving news about a positive HIV status can be devastating to a person and their loved ones. Coming to terms with it can take a great deal of time. Counseling is just one way our clients can use as a tool to move forward and take charge of their lives.
The counseling team also administers a range of mental health assessments and can provide referrals for special needs that exceed the scope of counseling services, including inpatient and outpatient treatment.
Insurance Assistance
The insurance assistance program pays for eligible health insurance premiums, co-pays and deductibles so that low-income HIV+ individuals can keep their insurance. Premiums will be paid directly to an insurance carrier or designated agent.
Although this program does not assist clients in obtaining new coverage, it is tremendously successful in preventing clients from becoming indigent.

ASNT's support and education groups are facilitated by our licensed professional counseling team, and vary in terms of schedule, location and focus according client need. Contact ASNT for more information.

Transportation
ASNT provides door-to-door transportation to medical and social service appointments for those clients who are unable to rely upon public transportation or travel by their own means. Transportation services ensures access to medical and other HIV related appointments for those with need.
This service is especially vital to clients in rural areas who must travel to other communities to receive quality health care and assistance with other needs.
